That it's only the css and images affected probably means the hacker doesn't have access to the machine per se (and can't change the database, for example), but only to part of it. I notice you have FTP access to your site. Maybe he knows the password, or maybe your wp-content directory is writable for other users on that machine? The permissions could have been changed to make installing wordpress plugins easier, or to change the theme from the web interface. Hope you figure it out. Yes, happened again.
